Affair with Married Woman

I am a single male in my 30s and have started an affair with a married woman. She keeps on saying she will leave her husband. Do you think I should continue with this affair?

I am not a sex expert nor am I an marriage counselor. But let me tell you some facts about relationships. 

Getting involved with married man or woman only brings bad news. The majority of married people who have affairs never leave their mates/sposes. 

She is only having an affair to spice up her life. Perhaps her husband is not paying attention to her, perhaps he is too busy and so on. But if she was really serious in leaving her husband, she would have done that prior to starting the affair with you. Any married person who tells you that he/she is planning to leave their partner soon and be with you- is a FUCKING liar. it will never happen.

This married woman is not interested in you at all. It is just the thrill of having an affair while being married that is exciting to her. Perhaps you are paying more attention to her. But this is only a transient thing. She has other goals and wants to play around-she may dump you and go out with other men. Who knows what is going on in her life.

You are young and should not invest any more time in this relationship. You have lots more to lose than her if the relationship breaks up. She will probably make up a cockamamie story and get back with her husband, and you will be old and left dry. Remember, when you have affairs with married people, life is not straight forward. You are on the constant look out, you cant go to certain places in case she gets caught. Most of your time with her will be spent in SEEDY motels, where she wont be seen by anyone. You will always be waiting for her-the relationship is not in your control. She controls everything because she is married.

Finally, no matter how pretty, handsome or rich the married partner is-the fact is that they have already deceived their partner by having this affair. So this person is already a liar. There is no reason, why this female will not continue the same trend when she is with you. So move on and find someone single-there are lots of nice single women out there ( i think!).

ECT and memory


Is memory permanently affected by electroconvulsive therapy?

This is not an easy answer. Some patients receive 8-12 sessions and have no memory loss and others undergo 2-4 sessions and have memory loss. On average, there is some memory loss in all patients after the procedure. The short-term memory is most affected.

If you have not recovered the memory after 12 months following ECT; it is unlikely you will recover it ever again. In any case, ECT is nothing like what it was in the past. It is a great procedure and can significantly improve the quality of life. The procedure is relatively safe and produces good results in most people. The stigma surrounding ECT from the 1950s continues to give it a bad rap, but the procedure is safe.

5-HTP for depression


I have mild depression and would like to take 5-HTP. What dose should I start?

5-HTP is a well-known dietary supplement that is extracted from a plant. In the body, it is converted to serotonin, which is thought to be a neurotransmitter for depression. Unfortunately, the dosing of 5-HTP is variable. Like all dietary supplements, there is no quality or quantity control. 

Some people recommend 50mg daily and others recommend 1000 mg daily, I would recommend start low and then gradually increase the dose. The increase in dose should be about 100 mg every week, to a maximum of no more than 1,200 mg per day. 

If you do not see any response by the end of 4-6 weeks, I doubt it will ever work.

Green tea fat burner

Is green tea fat burner effective for weight loss?

No, it is bullshit. A complete waste of money. It is only meant for stupid people who have money to waste. If you have any common sense, you should avoid this fraudulent product. Tea extracts with exotic names are now being marketed as a cure for everything underneath the sun. The only thing you will lose with green tea fat burner is the money in your pocket. 

Among the 6 billion people on this planet, you wont find one person who has lost weight with green tea. Give green tea a rest-it is a scam.

Medications and nightmares


What types of medications cause bad dreams or nightmares?

Almost every drug has the potential to cause bad dreams. In general, beta-blockers and almost all drugs that work in the brain have the potential to induce bad dreams. Unfortunately, this is an adverse effect of many medications and there is nothing anyone can do about it, except stop the drug. Best advice, take the lowest dose of the drug or you may ask your doctor to change the medication.

Over the counter medications for Acne


What over the counter medication can I use to treat my acne?

There is nothing over the counter that works. Salicylic acid, benzyl peroxide, topical antibiotics and all the soaps, are crap. Stop wasting your money on these fraudulent products-they do nothing.

If you have acne, go and see a skin doctor and get a script for a retinoid- ask for Retin A. It works, but it is expensive. If you are a female of childbearing age, the doctor may not always be willing to write you a script as the medication can cause birth defects. Therefore, you must have pregnancy test and cant get pregnant for at least 8-12 months, after stopping this medication. Nevertheless, it is the best drug for acne on the market.

Size of penis


Is there any medication that can increase the size of my penis?

No, there is no such medication. The only way to increase the size of your penis is with surgery. However, I should warn you that the surgery is very high risk and the results are abysmal. 

If you really want a bigger penis, I think you should first start by treating the problem in your head. Go and see a shrink and ask for Prozac. Mucking around with surgery and medications to increase the size of the penis will cause you more problems, than you ever imagined.

Sex and UTI


My GF often gets a UTI; can I get the bacteria when I have sex with her?

Females are more prone to UTI than males because of their anatomy. However, when you have sex with a female who has a UTI at the same, it is rare to acquire the bacteria from her. The bacteria do not survive outside the body and do not tolerate the pH of the semen well. 

However, males who have anatomical problems with their urinary tract can sometimes acquire these bacteria - it is rare. In general, females who have a UTI hate having sex at the same time because it is not exactly pleasant. 

The best advice is wear a condom if you are worried about acquiring any bacteria during sex.

Female with low sex drive


I am a 28-year-old female and have very little sex drive. Is there any thing I can take to reverse this problem?

So far, there is no thing approved by the FDA for low sex drive in women. My best advice is to first find out why you have a low sex drive. There are some who claim that low dose testosterone may help females with low sex drive. But before you start using this hormone, I should warn you that it can cause a variety of adverse effects like hair growth, masculine voice and so on. 

My best advice is to try Viagra. Even though it is not approved for women, there are ample reports that some women do find it useful. 

Perhaps your sex drive is low because your partner does not turn you on- maybe a physical change is needed in your lifestyle.

Diverticulitis


How can I know that I have diverticulitis?

Diverticulitis means inflammation of the colon. In most cases, it is the left colon that is affected. Without a CT scan, there is no way of telling. Clinical features include fever, pain on the left side and anorexia. However, a CT scan is usually required to confirm the diagnosis. 

Once the condition settles down with antibiotics and pain pills, you may need to undergo a barium enema or a sigmoidoscopy to look for diverticuli (small outpouchings in the colon). If you do have diverticulosis, the treatment is change in diet-meaning more fiber, avoiding constipation and lots of water.

Remember, surgeons will push you for surgery, but many people have diverticuli and surgery is not essential. If your diverticuli do not cause no symptoms (bleeding or pain), just avoid the surgeon.

Medications for genital warts


Are there any medications to treat genital warts?

Yes, there are several but they all require a prescription. Podophyllum and podofilox are two, which are widely used. They are usually applied by the doctor. Cure rates are low if they are used alone. These products work best when combined with freezing therapy. You may need to undergo several treatments over many weeks. 

These medication should not be applied on normal skin; otherwise, you can cause severe pain and burning. Generally, the normal skin is first covered with Vaseline and the medication applied on the warts. It does take several months to get rid of the wart. Recurrences unfortunately, are common if you only use the topical medication.

Genital warts


My doctor wants to treat my genital warts with trichloroacetic acid. Does it work?

Sometimes. This acid is less irritating and also has less toxicity compared to the other agents like podophyllotoxin. It has to be used many times, and even then, recurrences are common. If you are going to go with trichloroacetic acid, you need to combine it with freezing therapy. It is also the cheapest therapy and I do recommend it for genital warts.

Green tea for genital warts


Recently I heard that an extract from green tea could be used to treat genital warts. Is this true?

Yes, there is a product called veregen. It is an extract from green tea and used as a topical treatment for genital and anal warts. So far, there is not much known about this product and I cant tell you if it works. 

It does have the same type of side effects as podophyllotoxin. It should never be applied inside any body orifice but just on the external surface. It is an expensive product and if money is an issue, go with podophyllotoxin or trichloroacetic acid. 

Almost always, newer products are many times more expensive and are no more effective than the older medications.

Imiquimod for warts


Does imiquimod work for genital warts?

Yes, it does in about 50% of cases. Used alone, it is probably the most effective product. It is applied topically every other day for 1-2 weeks. The cream is applied and after 8 hours, it is washed away. The only downside is that it is prohibitively expensive. I would go with freezing therapy and podophyllotoxin.

Ganglion cyst


I have a ganglion cyst on my hand. Do I need treatment?

No, if it does not bother you, leave it alone. Most ganglions spontaneously disappear. Every technique for treatment of ganglion cyst is fraught with problems and recurrences. The best advice is avoid surgery unless you have severe pain. 

Ganglion cysts in most cases do not cause any symptoms and are of a cosmetic concern. Again, if it does not bother you, leave it alone. Complications are very common after surgery and this may include damage to your hand nerves and blood vessels. In some cases, the surgeon can mess up the ligaments of your wrist, which results in hand stiffness.

Treatment for OCD


I have been diagnosed with OCD. What is a good treatment for this disorder?

OCD is a difficult disorder to treat. In most cases, the disorder is treated with a combination of psychotherapy and medications. The latest antidepressants are useful but the disorder cannot be cured. In most people, the response to a drug often takes several months. I recommend you speak to your psychiatrist about the drugs. Psychotherapy alone is not effective for OCD and must be combined with medications.

Surgery for OCD

Is there surgery for OCD?

Yes, there is surgery for everything in medicine. When there is money to be made, doctors will always invent a surgical procedure-rest assured. The surgery for OCD in simple terms is Bullshit. It is a crap. 
Deep brain stimulation is done by inserting electrodes into your brain and uses a pacemaker to stimulate the nerves. The treatment is not only expensive ($20,000-$40,000) but there is no guarantee it works. In addition, some of the complications after surgery are serious. You may not have OCD anymore, but afterwards you may not be able to speak, walk, eat or poop. 

Leave surgery alone- it is still experimental and the results are abysmal. If you have money to spare, give it to the local SPCA or take a long vacation. Leave the surgery for gullible fools who have money to throw away.

Surgery for low back pain


I am a 73 year old male and pretty healthy. I recently had an MRI which showed spinal stenosis. My doctor says I need surgery. Any suggestions?

At your age or any age, surgery for spinal stenosis is not recommended. The results of surgery on the back are horrible. You will have a miserable life. Please do not rush in for surgery. 

I would first recommend physical therapy for 6-9 months. The one thing you should do is keep away from these back surgeons. Most of them have skills like a butcher and others just “slash and dash.” The majority of people who undergo back surgery are worse off. 

So if you want to live a life without hassles, start some type of exercise and omit the word surgery from your vocabulary.



Irritable bowel syndrome and belching


I have irritable bowel syndrome. I belch a lot. Should I try out a gluten free diet?

In the last few years, gluten free diets are being hyped as a cure for all digestive problems. Part of the problem is that the manufacturers of these gluten free products are preying on desperate people with IBS. 

Gluten free products are very expensive and usually the diet is very restricted. Gluten free diets are meant for patients with celiac disease. If you do not have celiac disease, then stop wasting your money on these products; they will not help you. Of course, there are always some people who will claim otherwise. However, these same people will also claim they have benefit after eating dog’s poop.

People without celiac disease who consume gluten free diets have serious mental issues and their anecdotal reports are not scientific proof that this diet works in everyone. If you really are keen on a gluten free diet, try it out and see what happens. If your symptoms still continue after a few weeks, I would not continue this diet.

Home treatment for osteoarthritis


I have osteoarthritis of my knee but do not want to take any medications. Is there any home treatment that is cheap?

Sure, there are many things you can do to relieve OA pain.

First, lose weight and enroll in a physical therapy program. If you cant afford PT, just go to the local pool and walk in water. Do it 2-4 times a week and you will quickly see the difference. Physical therapists have become rip off artists like doctors, charging hundreds of dollars per hour for routine common sense exercises. So avoid them. 

I do not recommend glucosamine or chondroitin because there is no proof that they work-another rip of marketed by orthopedic surgeons. 

Use an ambulatory device to support and maintain balance. 

Acupuncture is reported to decrease pain, but you have to go for many sessions. Each session costs $75-$150 and it does not work in everyone and the pain relief is mild. By the time you are done, the acupuncturist will have paid for his/or Honda. Avoid this BS therapy.

Eat right and walk regularly.

Torn anterior cruciate ligament


I am a 24 year old male and just tore my anterior cruciate ligament. I am not an active sportsperson. What is the treatment?

In general, for active sports people, the treatment for torn ACL is reconstructive surgery, followed by intense rehabilitation. Unfortunately, the orthopedic surgeons regularly push everyone for surgery- chiefly for financial gain. 

Now the thinking is that for non sports people with partial tears, conservative therapy can work just as well. Surgery is always an option if this fails to heal the ACL. 

My advice is to enroll for physical therapy for at least 6-12 weeks. Wear a knee brace to keep your knee stable. While surgery has it benefits like a more stable knee right away, it is more expensive and the overall results are not that different from conservative therapy. To learn what exercise to do at home, just go online and search for physical therapy for torn ACL. Good luck.

In grown toenail


How can I get treatment for my ingrown toe nail?

Ingrown toe nails can be very painful and this is one reason why you should regularly trim your nails. The treatment of ingrown toe nails depends on severity. For mild cases, soak your foot in warm water several times a day. By gently massaging the skin on the side of the toe nail, you can sometimes free the edge of the ingrown toe nail and cut it. 

However, in most cases, the toe is red and painful. In such cases a visit to the doctor may be required. The doctor will usually numb the toe and cut the side of the nail. In rare cases, the entire nail must be removed. To prevent ingrown toe nails, wear shoes with wide toes and keep the skin clean and dry. Women who wear high heels and pointed shoes are most prone to ingrown toe nails.

Jock itch


What exactly is  jock itch? I am always getting the urge to scratch my groins and my doctor told me I had jock itch.

Jock itch is a fungal infection of your groin. The itching sensations can be intense and continuous. In many cases, if left untreated the red rash can spread from the groins into the buttock and anal area. Jock itch tends to flourish when the groin is wet and humid; and especially when you wear tight under garments. 

There are several creams and powders you can use to treat the jock itch. The powder is applied twice a day for several weeks or even months. The fungus can be spread by your fingers, so wash your hands thoroughly after applying the medications. 

To avoid jock itch, keep the groin area dry and clean. Dry your self after a shower and wear loose cotton under garments. Do not share personal care products with others.

Candida


Are there any over the counter products to treat candida?

Sure there are many products. Just ask the pharmacist. These products are available as creams, suppositories, tablets and ointments. In some women, a single dose of diflucan can also helpful. To prevent future yeast infections try and keep the perineum clean and dry.  Avoid the use of irritating soaps and sprays. Change your under garments frequently and wear loose cotton underwear. Never take antibiotics for prolonged periods unless told by your doctor.

Menopause and painful sex


I am a 45 year old female and have just started menopause. I now find sexual intercourse quite painful. It never used to be like that. What can I do about it?

During menopause, the vaginal secretions diminish. In addition the  vaginal tissues loosen and become thin. This often results in painful intercourse. The pain can vary from mild to severe. However, there are several treatments you may want to try. Use a vaginal estrogen cream. There is a flexible vaginal ring which is placed inside the vagina and it releases low doses of estrogen. It takes a few weeks to get the full effect but it will make sex better. Also use lubricants liberally during sex. Lubricants help decrease friction. 

I do not recommend the use of lidocaine which is a local anesthetic. While this chemical can take away the pain, it also numbs the pleasurable sensations. Finally, you may want to try intercourse with you on top of your partner. This position is less painful than the routine missionary position.

Sex in Aruba


I am a 22 year old male and was in Aruba several weeks ago. The night life led to sexual encounters with several females. I am back in NY and have noticed a discharge from my penis. Is this always a sign of an infection?

In general, unlike women, men do not normally have penile discharge. So if you have a penile discharge, especially since you just had a sexual encounter, it is very likely that you have acquired an STD. The color of the discharge is irrelevant. I suggest you go to your doctor and get tested for an STD. I guess  a condom was not available or you were too cheap to buy one!

STD after affair


I am a 25 year old married male and recently had an affair with a female co-worker. I have now been diagnosed with an STD. Should I tell my partner?

If you have an STD, it is an obligation to tell your partner, esp your wife. The reason is that she can be treated if you have transmitted anything to her. I highly recommend you bring in your wife next time you see the doctor and explain the whole situation to your wife. Let me tell you another thing- in many states, having an STD and not informing the partner before sex can result in legal hassles for you. Your STD will be cured, but I am not sure about your marriage.

Peels for acne

Can my acne be treated with an acid peel?

Absolutely not. Chemical peels are a poor man’s way of undergoing 
cosmetic procedures. These peels are useless, expensive and have a range of side effects. For acne, these peels can make your skin look worse.

Acid peels have often been used to treat sun damaged skin, remove pigmentation spots (freckles, sun spots), melasma and fine lines. The acid peel acts by removing the most superficial layers of the skin. This results in an even skin surface with bright color and a softer texture. Acid peels are frequently used by therapists or nurses in beauty clinics.

The higher concentration peels are generally applied by physicians. The milder acting peels can be applied once every 3-4 weeks but the more concentrated peels are applied once every 4-6 months. The most common acid peels used today include glycolic acid, salicylic acid, phenol, and trichloroacetic acid.

The mild acids are a waste of money because they are no better than urine (Piss).

The higher strength peels do work but can make a mess of your skin.

If you have any common sense, stay away from chemical peels, unless you want to look like Godzilla/gorilla. If you have acne, go and ask your doctor for some Retin A.

IPL Photofacial

Can IPL photo facial remove my wrinkles?

No, it cannot.

Intense pulse light therapy (IPL) has been devised to rejuvenate the face, treat acne and/or erase the fine lines. IPL is no different from the other laser therapies. It is used to treat many skin disorders but only produces good results form minor skin blemishes.

IPL photo facial is expensive and you need to undergo many sessions. Each session can cost anywhere from $300-$600 dollars. The light treatment may erase some fine lines and skin imperfections but do not expect your scars or wrinkles to go away. Even when it works, the results of IPL are not instant and often take weeks.

If you have the money, try the therapy but be realistic. If you do not have the money, avoid smoking and maintain good skin hygiene. In addition, remember, all of us get old and develop wrinkles. IPL photofacial is only a temporary solution and not a cure for aging

Cosmelan

Recently I was told to buy Cosmelan to reduce my dark skin patches. Does it work?

No, it is bullshit. Cosmelan has been hyped to reverse many skin pigmentation disorders but in fact it is useless. The cosmelan pack contains mild chemical peels, koji acid, plus some bogus vitamins. All it does is cleans your face and removes the junk. It is an expensive product and definitely not worth the money.

You can do the same thing with a bland soap and water. Cosmelan has to be applied regularly for many weeks before you see a result-most people see nothing.

I definitely do not recommend this crap as it is nothing more than a scam. Many online reviews indicate that the product does nothing. So even if you have money to throw away, don’t fall for this crap. Rather, go for a vacation and enjoy your hard earned money. If you do not like a vacation, buy some food for the animals at the SPCA.
